Why It Matters
The first warning shouldn't be the emergency room.
of Illinois adults now live with diabetes, up from under 10% a decade ago.
Illinois Department of Public Health / CDCof Illinois adults live with more than one chronic condition.
CDCA visit at the library on a Tuesday evening can change the whole story.
How our clinics are builtThe pattern is familiar: no time off work, a trusted clinic that closed, a checkup that keeps getting pushed. High blood pressure and diabetes are the quiet drivers behind most preventable emergency room visits, and both are easy to catch.
A Day at the Clinic
What a visit can look like
A 54-year-old warehouse worker has diabetes and high blood pressure. He hasn't seen a doctor in three years. The last time anyone checked his blood sugar was in an emergency room. He works second shift, and the one clinic he trusted closed during the pandemic.
The Prism adult health van parks at his neighborhood library on evenings he's off. In one stop: blood sugar checked, blood pressure measured, an eye exam referral for his diabetes, a take-home colon cancer screening kit, and a plan to get him a regular doctor.
One visit. On his schedule. That's healthcare that actually fits a working life.
Common Questions
What our neighbors ask us.
How much does it cost?
Most services are free at community events. No one is turned away.
Do I need insurance?
No. You don't need insurance to be seen, and we don't require it to help you.
Do I need to skip breakfast for the tests?
No. Our main checks (blood pressure and A1c for blood sugar) work fine even if you've eaten. If any follow-up test needs fasting, we'll tell you exactly what to do.
When do I get my results?
Many results come back on the spot, while you're still with us. For lab work that takes longer, we contact you directly and explain what it means.
Can Prism be my regular doctor?
We're a community clinic that comes to you, and one of the most useful things we do is connect you with a regular doctor near you for everything in between our visits.
I work all day. When can I come?
Many of our events run evenings and weekends on purpose: libraries after work, weekend health fairs, and workplace visits. Check the events calendar for times near you.
It's been a while? That's exactly who we're for.
