We train, credential, and launch community health workers and doulas, then stand behind them with curriculum, supervision, and a place on real care teams. It's the same model that built the state's only 24/7 hospital doula workforce, at Stroger Hospital with Cook County Health.

Memory loss changes life for the whole family. Prism supports families and caregivers navigating dementia, with plain-language education, caregiver support, and connection to community resources, brought to the places people already are.
